Limited Accident and Sickness Plan Options
There are 3 plan design options to choose from. The plans all cover the same services, but differ in the amount of indemnity benefit that the plan will pay for those services. The 3 options are known as the Value Plan, the Standard Plan and the Enhanced Plan. Additionally, the Generic Prescription Drug Card can be added to any of the plans.
All plans offer:
- Guaranteed issue coverage for eligible employees and their dependents - no medical evidence required
- 6 month pre-existing condition limitations on hospital and surgery benefits only
- No coordination of benefits regardless of any other coverage in force
- Employee-only and Family Plan Options
- Employer-paid or voluntary 100% employee-paid options
- Easy and convenient payroll deduction
- Assignment of benefits directly to service providers
In addition, all plans include the following value added services for employers and employees:
- Access to Beech Street's PPO network provider discounts and national network for discounts on prescription drugs
- Toll-free numbers for customer service including inquiries and claim service
- Multilingual claims customer service capabilities-English and Spanish on a standard basis; other languages available if necessary to better serve your employee population
- 24-Hour Nurse Hotline provides around-the-clock access to detailed health information and the ability to speak with a live health professional
- Administration of COBRA benefits for group with 20+ employees
