- Physician Office Visits: Up to 5 visits per Plan Year
- Outpatient Lab & X-ray: Up to 3 visits per Plan Year

- Hospital Confinement: Up to 100 days per Plan Year with double the benefit for ICU confinement up to 30 days per Plan Year
- Surgery: 1 Inpatient and 1 outpatient surgery per Plan Year
- Preferred Provider Organization (PPO): Access to the Beech Street National PPO for even greater savings. No penalty for choosing an out-of-network provider!
